- You are over 18 years old.
- You are registered with a GP practice in North West London (Brent, Harrow, Hillingdon, Hounslow, Hammersmith & Fulham, Westminster, Kensington & Chelsea and Ealing).
- You have been diagnosed with cardiovascular disease.
However, anyone registered with a GP practice in North West London can create an account and access MyHealth London resources and online learning modules.
